Abstract |
Objective The aim of this study is to present the modified classification of spinal cord vascular lesions by Spetzler RF and treatment outcome of lesions.
Methods From February 1999 to April 2004, eleven patients with the spinal cord vascular lesions were treated in our hospital. The lesions confirmed by the spinal angiography or pathological diagnosis were included in this study. The most recent modified classification presented by Spetzler RF was applied. Three major types of lesions are introduced: neoplastic, aneurysmal, and arteriovenous lesions.
Results Eleven vascular lesions were operated or embolized except three cases. Lesions are divided into three broad categories : neoplasms, aneurysms, and arteriovenous lesions. Neoplastic vascular lesions included hemangioblastomas(2 cases), cavernous malformations(2 cases). There were not a case of spinal aneurysms. Spinal cord arteriovenous lesions are divided into arteriovenous fistulas and arteriovenous malformations. Arteriovenous fistulas are subdivided into extradural, intradural lesions. Intradural lesions are categorized as dorsal(4 cases) or ventral(2 cases). Arteriovenous malformations are subdivided into extradural-intradural and intradural malformations. Intradural lesions are further subdivided into intramedullary(1 case), intramedullary-extramedullary, and conus medullaris.
Conclusion This modified classification gives us a good understanding for the spinal vascular lesions and can be used to determine the appropriate treatment.
