Direct Repair of the Pars Interarticularis Defect using Transpedicular Screw and Laminar Hook: Preliminary Report

Abstract
Objective
The authors utilized the technique of direct repair of pars interarticularis defect using transpedicular screws and laminar hooks for the patient with symptomatic lumbar isthmic spondylolysis.
Methods
The procedure was performed in 10 patients(11 segments) who had bilateral defects of the pars interarticularis including 3 patients with Meyerding grade I spondylolisthesis, all of them had been unresponsive to conservative treatment. X-rays and computed tomograms were taken during follow-up period to confirm whether bony fusion has been achieved or not. All patients were evaluated by MacNab's criteria at average 17.9 months.
Results
Nine of 10 patients had treated by this method showed the bony union of pars defect and significant pain relief. The patient with severe disc degeneration and spondylolisthesis of 5mm showed nonunion of pars defect and unrelieved back pain. There was no case of instrumental failure. Clinical results were excellent in 6 patients, good in 3 patients, and fair in 1 patient.
Conclusion
Our cases demonstrate that direct pars repair is a safe, functional and effective modality to treat selected patients with spondylolysis or low-grade spondylolisthesis. Careful patient selection is crucial for successful outcome, especially for the patients older than 30 years. Further follow-up study is necessary to establish the long-term outcome and stability.
